Job DescriptionDescription for Internal Candidates:
1.
Assesses the patient's physiologic health status oConducts patient interview, explains policies and procedures to patient/significant others, reviews patient's chart and answers questions correctly and courteously; oAssesses gastrointestinal, cardiovascular, respiratory, renal and neurological health status; oDetermines mobility, sensory deficits, prostheses use, and skin condition and adjusts plan of care, as appropriate; oAssesses level of pain and pain management; oCommunicates/documents patient's physiologic health status and plan of care.

Assesses patient's psychosocial health status oElicits perception of medical/nursing care and the expectation of care; oDetermines coping mechanism, knowledge level, and ability to comprehend; oIdentifies cultural/ethnic requirements to reduce anxiety and ensure patient satisfaction; oCommunicates and documents psychological status and care plan; support group, counseling services, social services.

Identifies patient outcome oDevelops criteria for measurement of patient outcomes; oIdentifies actual/potential patient problems; oIdentifies patient's need for teaching based on psychosocial and developmental assessment; oDevelops patient outcome statement (s) and establishes individualized patient goals.

Formulates the plan of care and establishes the priorities necessary to achieve expected outcome oIdentifies care activities and establishes the priorities necessary to achieve expected outcome; oCoordinates the cost-effective use of supplies, equipment and medication to achieve expected outcome; oDocuments the plan of care and collaborates with physicians and other health team members.

); oExercises professional skills related to the plan of care; oReassesses patients as needed and appropriately revises plan of care; oCorrectly administers prescribed treatments; oCorrectly uses equipment necessary for patient care; oProvides emotional support to patient and significant other; oApplies scientific principles in performing procedures; carries them out safely, timely and efficiently; oMakes accurate observations of patient's conditions during treatments/procedures.
Reports and records same as appropriate; oKeeps accurate documentation of patient's treatment, activity and condition, as well as patient's responses to medical and nursing interventions; oUses appropriate methods of documentation according to departmental policy; oActs rapidly and effectively during any emergency situation, managing self, patients and other employees; oProvides a calm, quiet, restful atmosphere.
Communicates effectively with the patient's family or caretakers; oParticipates in planning for discharge and coordinates referrals, as appropriate; oProvides discharge instructions to patient and significant other.

Evaluates care provided for patient outcome oDemonstrates ability to measure effectiveness of care provided and documents same; oPerforms variance analysis related to outcome data for performance improvement; oDesigns, implements, and evaluates systems to improve care in Unit; oKeeps accurate documentation of patient's treatment, activity and condition; oUses appropriate methods of documentation according to departmental policy.

Collaborates with other care team members in planning and carrying out treatment regimen; provides direction to other members of the care team oCollaborates with the appropriate physician on patient's plan of care; oAccurately interprets and implements treatment regimen as prescribed by the Physician; oAssists the Physician during treatments and/or diagnostic procedures; oKeeps the Patient Care Manager/designee and/or physician, abreast of changes in patient's condition and/or treatments, as appropriate; oUses clinical judgment in delegating assignments to the Patient Care Associate, Support Care Associate and Clerical Service Associate in providing patient care, and ensures that assignments are completed in a timely fashion.

Performs grade I-IV Decubritus Care oPerforms preventive skin care measures oApplies simple dressings, maintaining principles of aseptic technique; oApplies warm & cold compresses, consistent with facility procedure; oPerforms irrigations, consistent with Facility procedure; oPerforms other procedures related to skin care, as necessary.

Administers medications correctly and safely oCorrectly identifies medication:
action; dosage; side effects and implications for the patient; oMeets standard on medication administration examination; oDemonstrates preparation of local solutions; oDemonstrates preparation of I.

drips; oAdministers and documents medication correctly; oAdministers IVP medication as per department policy and procedure; oEducates patients and significant others related to drug and food interactions.

Participates in patient and family education oProvides the patient with an explanation of his/her condition as indicated; oCommunicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies; oRecognizes and utilizes health teaching opportunities and resources /materials available for this teaching; oProvides for early discharge planning and appropriate referrals for post-hospital care; oEvaluates the effectiveness of teaching by feedback from patient/family and documents same.
11.
Performs related duties, as required.
PHYSICAL DEMANDS:
Constant - standing, walking, stooping, bending, stretching, lifting, pulling, pushing and turning.
Frequent - Lifts objects from 10 lbs to 150 lbs.
Pushes objects up to 200 lbs.
Occasional - Sitting.
Lifts with assistance objects greater than 150 lbs.
and carries objects weighing up to 50 lbs.
ADA Essential Functions Job Qualification:
oGraduate from an accredited School of Nursing.
Bachelor's Degree in Nursing, preferred.
Must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010 and obtain a BSN degree within five (5) years of employment date.
oCurrent license to practice as a Registered Professional Nurse in New York State.
