A healthcare network in New York City is actively seeking a licensed Registered Nurse (RN) to join their growing staff on Staten Island as a Case Manager.
In this role, the Case Manager (RN) will be responsible for developing plan of care and coordinating the delivery of healthcare services for persons receiving services.

Responsibilities:
The Case Manager (RN) will:
Coordinate the delivery of routine and specialty healthcare services to address assessed needs of persons receiving services in assigned program unit.
Assess and establish health care outcomes for persons receiving services Develop plan of nursing services and oversee the implementation of the healthcare plan by educating person's receiving services, DSP/Caregivers and families in the plan of nursing services Educate, monitor and supervise staff in carrying out ordered treatment plan related to health needs Evaluate the effectiveness of preventive/ongoing healthcare and make recommendations / referrals for alternative care, as necessary Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers Complete and maintain all required documentation on the approved electronic health record system Conduct general and individual specific health related trainings for persons receiving services, direct care staff, and other team members, as needed.
Perform all other duties as assigned
Qualifications:
Graduate of an accredited School of Nursing NYS Registered Nurse (RN) license Solid assessment, clinical and documentation skills Patient oriented Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized Desired Skills:
Bachelor's Degree in Nursing Experience working with persons with Developmental Disabilities Recommended Skills Attention To Detail Certified Nurse Practitioner Clinical Works Communication Coordinating Developmental Disabilities Estimated Salary: $20 to $28 per hour based on qualifications.
